Biliary-Digestive Derivations in the Twenty-Forst Century

Abstract

Introduction: Technological advancement introduced less invasive derivative methods for treating biliary obstruction (endoscopic retrograde cholangiopancreatography, percutaneous access, and echoendoscopy); however, in settings with limited technological resources, it is necessary to resort to biliary derivative treatments by conventional approach.

Objective: To present the latest advances in the use of biliary-digestive derivation. A narrative review with critical and integrative intention was carried out, starting from a search of the PubMed, SciELO, Bireme, Scopus, Web of Science, LILACS, EBSCO Dynamed, Springer, Cochrane databases, as well as the Google Scholar search engine, prioritizing English and Spanish in the last 10 years. This made it possible to know that the obstacle to biliary flow is the phenomenon responsible for liver failure, renal failure, sepsis and death, if not treated timely. The priority of its treatment lies in the permanent decompression of the obstructed biliary tract with curative or palliative intention, through the use of biliary-digestive derivations in its different conventional, endoscopic and percutaneous accesses; and then to conclude that the knowledge of the different approaches to perform a biliary-digestive derivation allows the selection of the ideal access according to the individual clinical characteristics of each patient with extrahepatic biliary obstructive disease.
