Endoscopic Ultrasound in the Differential Diagnosis of Pancreatic Lesions

Introduction: Endoscopic ultrasound is the most commonly used procedure to determine the etiology of pancreatic lesions.
Objective: To determine the utility of endoscopic ultrasound in the differential diagnosis of pancreatic lesions.
Methods: An observational, descriptive, cross-sectional study was conducted (January 2016–October 2023). The main variables analyzed were: endoscopic ultrasound diagnosis and histological diagnosis. Descriptive summary statistics were used for statistical analysis according to the type of variable. The chi-square test (X²) and Fisher’s exact test (p < 0.05) were used to compare variables between the two groups. Agreement between the endoscopic ultrasound and histological diagnoses was measured using Cohen’s Kappa coefficient.
Results: Of 48 patients, 32 (66.7%) had malignant neoplasms. Among the ultrasound findings, location (p = 0.05), clarity of borders (p = 0.01), regularity of margins (p = 0.00), echo pattern (p = 0.025), dilation of the main pancreatic duct (p = 0.03), and number of passes (p = 0.00) were statistically significant. There was agreement between the endoscopic ultrasound diagnosis and the histological diagnosis (Cohen’s kappa coefficient: 0.847 with an associated probability of 0.000).
Conclusions: The location of the lesion in the pancreatic head, ill-defined borders, irregular margins, dilation of the main pancreatic duct, and a heterogeneous echogenicity pattern are endosonographic features associated with malignancy. Endoscopic ultrasound is useful for the differential diagnosis of pancreatic lesions and may be the first-line method to use in these patients.
