Abstract

Introduction: Several classifications of inguinal hernias have emerged in recent decades, but none has unified pathological anatomy criteria, functional criteria, and guidelines for surgical treatment. A new clinical-surgical classification has been developed and applied, which allows for the planning of surgical treatment based on risk factors.

Objective: To evaluate the results of implementing a new clinical-surgical classification for inguinal hernias.

Methods: A quasi-experimental study with a non-equivalent control group was conducted in the General Surgery Department of the Dr. Juan Bruno Zayas Alfonso General Hospital in Santiago de Cuba from January 2018 to December 2022. Two groups of patients were compared, and variables such as probability of recurrence, age, sex, use of bioprostheses, exposure to heavy physical exertion, and place of origin were studied.

Results: A new clinical-surgical classification for hernias is presented, which revealed a predominance of bioprosthesis use in the preoperative assessment (87.3%) and during surgery (86.8%). Bioprosthesis techniques were used in all patients exposed to heavy physical exertion; meanwhile, in those not exposed, they were used only when the patient presented another risk factor.

Conclusions: The implementation of the new classification guides and refines surgical treatment by incorporating the use of bioprostheses, particularly in patients with small inguinal hernias and the presence of any other significant risk factor.
